Job Description
**Responsibilities**
+ Patient population includes Geriatrics, Dementia, and Palliative Care
+ Collaborate closely with interdisciplinary teams and Advance Practice Providers to enhance efficiency and care coordination to ensure comprehensive patient care
+ Actively engage in patient education and shared decision-making to empower older adults and improve health outcomes
+ Serve as a Geriatrics Consultant, including screening and diagnosing Dementia
+ Clinic position with possibility of home visits
+ Full time position, Monday through Friday
+ Provider will have 4 hours of administrative time per week
+ Daily patient volume: anticipated 6-10 patients per day
+ Current programming includes multi-disciplinary Dementia Screening Clinic, Alzheimer's & Dementia Care Program (UCLA dissemination site), Outpatient and Inpatient Palliative Care, and GUIDE model participant
+ Opportunity to enhance and expand services in the Cunningham Center for Compassionate Aging
**Qualifications**
+ BE/BC in Geriatric Medicine (IM or FP), Neurology, or Hospice & Palliative Medicine (IM or FP)
+ Experience or interest in working with Dementia/Alzheimer's Disease population
+ Strong communication and interpersonal skills
+ Valid state medical license as MD or DO
+ Valid DEA certificate
+ Strong clinical skills and experience working with older adults
+ Ability to work effectively in a team-based healthcare environment
+ Demonstrate empathy, compassion, and respect for the unique needs of older adults and their care partners #HEC

**Pay Range**
$120.20 - $134.62 /hour)
