Qualifications
Required

• Associate degree or higher

• Valid Registered Nurse (RN) license in the state of Illinois (IL)

• Valid Certified Cardiac Rehabilitation Professional (CCRP)

• Valid Regular Driver’s License (DL)

• All of the following valid certifications:

  • American Heart Association Basic Life Support (AHA BLS)

  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)

• Willingness and ability to travel as required for this role

Preferred

• Please provide your RN License Number (required for name clearance)

Overview
Our Award Winning Client is currently seeking a committed and experienced Weekend Start of Care (SOC) Nurse (RN) to join their team. This full-time position plays a critical role in promoting coordinated, high-quality care by conducting patient assessments, implementing care plans, and administering direct nursing services across the Gurnee, IL area and surrounding regions. Candidates must be skilled in managing home health admissions, communicating effectively with healthcare teams, and maintaining accurate, timely documentation.

Key Responsibilities

• Complete start of care assessments and documentation for new admissions and post-hospitalization patients

• Assess eligibility and incorporate telehealth into the care plan, notifying the telehealth department promptly

• Develop and coordinate individualized care plans in collaboration with other agency professionals

• Monitor, document, and report patient responses to treatments and changes in condition

• Educate and support patients and families regarding care plans and safety procedures

• Assign and direct Home Health Aides based on care needs and agency guidelines

• Maintain up-to-date documentation that meets regulatory and organizational standards

• Participate in ongoing training, CEUs, and performance improvement activities

• Communicate clearly with supervisors and team members to ensure continuity of care

• Maintain daily contact with scheduling/supervisory staff and report any changes

• Adjust to staffing needs and changes in daily schedule as required

• Optimize travel routes for efficiency and maintain expected productivity

• Maintain professional behavior and uphold patient rights and agency compliance standards

• Must meet weekly productivity goals: equivalent of three (3) SOCs or ROCs per day, totaling six (6) points

• Participate in holiday rotations as required

• Other duties as assigned by supervisor

Schedule and Shift Details

• Every Saturday & Sunday, plus two additional weekdays for a total of four 10-hour shifts

• Preferred schedule: Friday – Monday or Saturday – Tuesday

• Orientation required: Two weeks, Monday–Friday (8-hour days)

Travel

• Local travel required throughout Gurnee and surrounding areas

• Travel expectations are based on patient needs and will remain within a reasonable territory whenever possible
